Winning Edge Basketball Waiver

By completing this form, you acknowledge you are the parent or legal guardian of the athlete you are signing up and are agreeing to the following:

Financial Responsibility: "The parent or legal guardian" agrees to the financial responsibility of their athlete for any and all costs associated with Winning Edge Basketball including practice rentals, practice costs, league costs, tournament costs, and any other team related costs during the athlete’s enrollment with Winning Edge Basketball..

Waiver and Release: "The parent or legal guardian" agrees to indemnify and hold harmless Winning Edge Basketball, it’s coaches Aidan Carroll, Matthew Blose, and any parent voluntarily acting as a coach from and against any and all lawsuits, awards, claims, damages, liabilities, costs, expenses (including reasonable attorney fees and related costs) arising out of injury or damages to your athlete or any participant in connection with "the participating minor's" participation in Winning Edge Basketball events 01/31/2025 until 12/31/2025.
